Instructions:
Step 1 - Complete the form below with your credentials, and uploading both your current passport and your Polynesian Union passport photo

Step 2 - Email us your biometric info (prints) with signature using the pdf template from this page.

Step 3 - After processing and approval, your Ambassador passport will be issued and we will ship to the address of your choice.
